Service: Saturday, March 6, 2015 @ 1 p.m. at Mt. Moriah M.B. Church

Visitation: Friday, March 6, 2015 from 4-6 p.m. at the funeral home

Interment: Mt Moriah Church Cemetery

He is survived by five sons, Charlie L. Polk (Belinda), Aberdeen, James O. Polk (Dorothy), Amory, Robert L. Polk, Tupelo, Michael W. Polk (Marie), Huntington, TN, two sisters, Loraine Clay, Baton Rouge, LA and Shirley Polk, and three sisters, Eva Mae Jamison, Huintington, TN, Maurine Fischer, Indianapolis, IN, and Arnetta Gordon, St. Lopuis, MO
